What is Olarigen 150 mg?

Olarigen 150 mg is a generic formulation of Olaparib — a first-in-class PARP (Poly ADP-ribose Polymerase) inhibitor — manufactured by General Pharmaceuticals Ltd. under cGMP compliant conditions in Bangladesh. Each tablet contains 150 mg of olaparib, the same active molecule found in Lynparza® (AstraZeneca), and is dispensed under the Named Patient Program for patients requiring a quality-assured, affordable alternative.

Olaparib targets tumour cells with homologous recombination repair (HRR) deficiency, most commonly caused by BRCA1 or BRCA2 mutations, using the principle of synthetic lethality.

How Olaparib Works

Olarigen blocks PARP enzymes (PARP1, PARP2, and PARP3) in your cells. PARP plays a key role in repairing damaged DNA. Cancer cells with specific mutations, such as BRCA1 or BRCA2, already rely heavily on PARP to survive because their other DNA repair pathway is impaired.

When Olarigen blocks PARP, BRCA-mutated cancer cells cannot repair their DNA and undergo cell death, while healthy cells — which retain functioning backup repair pathways — are generally spared. This selective mechanism is known as synthetic lethality. NCCN guidelines list olaparib as a Category 1 recommendation for BRCA-mutated ovarian, breast, prostate, and pancreatic cancers. What to Expect: First 30 Days

Olaparib is generally well tolerated for a maintenance therapy, though gastrointestinal symptoms and fatigue are common in the early weeks.

  • Week 1: Nausea is common when starting therapy. Eating small, frequent, bland meals (like toast or crackers) and avoiding greasy or spicy foods can help significantly.
  • Weeks 2–3: You may feel unusually tired — prioritise rest and try light activities like short walks to maintain energy levels. Blood work begins to be monitored closely during this window.
  • Week 4: A baseline Complete Blood Count (CBC) and ongoing monthly CBC testing for the first year is essential to detect any drop in haemoglobin or white blood cells early.

Side Effects

Most side effects are manageable, but olaparib carries rare but serious risks that require ongoing monitoring throughout treatment.

Common · Usually Manageable

  • Nausea and vomiting
  • Fatigue
  • Anaemia (low red blood cell count)
  • Decreased appetite
  • Diarrhoea
  • Headache
  • Altered taste

Serious · Report Immediately

  • Myelodysplastic syndrome (MDS) / acute myeloid leukaemia (AML)
  • Pneumonitis (lung inflammation)
  • Severe or persistent low blood counts
  • Venous thromboembolism (blood clots)
⚠ Call your doctor immediately if you experience:
  • New or worsening breathlessness, dry cough, or fever (possible pneumonitis)
  • Unusual bruising, prolonged bleeding, or signs of infection (possible blood disorder)
  • Sudden leg swelling/pain, chest pain, or breathlessness (possible blood clot)

How to Take Olarigen 150 mg

Standard dose: 300 mg (two 150 mg tablets) twice daily, for a total of 600 mg per day, with or without food.

  • 1
Two tablets, twice daily — approximately 12 hours apart, e.g. morning and evening.
  • 2
Swallow tablets whole with a glass of water. Do not chew, crush, or dissolve them.
  • 3
Avoid grapefruit and Seville oranges (including marmalade) — these significantly increase olaparib blood levels via CYP3A4 interaction.
  • 4
If you miss a dose, take your next dose at the scheduled time. Do not double up.
  • 5
Storage: below 30°C in a dry place, protected from sunlight, windows, or humid areas like bathrooms.

Caregiver Guidance

  • Track blood test appointments closely — a baseline CBC plus monthly testing for the first year is essential to catch low blood counts early.
  • Support nausea management — encourage small, frequent, bland meals and avoid greasy or spicy foods, especially in the first weeks.
  • Watch for fever or unusual bruising — report any signs of infection, unexplained bruising, or prolonged bleeding to the oncologist promptly.

If the Medicine Stops Working

Resistance to olaparib can develop through restoration of homologous recombination repair function — for example, through secondary BRCA mutations that reverse the original mutation, or upregulation of drug efflux pumps that reduce intracellular drug levels. If disease progression is detected, your oncologist will reassess treatment options based on the specific clinical setting, which may include chemotherapy, a different PARP inhibitor, or a clinical trial.

Frequently Asked Questions

How does Olarigen 150 mg work against cancer?
Olarigen works by blocking a protein called PARP in your cells. PARP helps repair damaged DNA. Cancer cells with specific mutations (like BRCA) rely heavily on PARP to survive. When Olarigen blocks this protein, the cancer cells cannot repair their DNA and die, while healthy cells are generally able to use other repair methods to survive.
Who manufactures Olarigen?
Olarigen is manufactured by General Pharmaceuticals Ltd. in Bangladesh, a company that emphasises adherence to cGMP (Current Good Manufacturing Practice) guidelines for its oncology product line.
How should I store Olarigen tablets at home?
Store Olarigen below 30°C in a dry place, away from direct sunlight, windows, or humid areas like bathrooms. If your home gets very hot, consider storing the bottle in a cool, dark cupboard or a dedicated medication cooler.
What monitoring is required while taking this medication?
Your oncologist will require a Complete Blood Count (CBC) test before starting treatment and monthly for at least the first year, to monitor for side effects like anaemia or neutropenia. Renal function (kidney tests) will also be checked periodically.
Is Olarigen 150 mg the same as the 50 mg formulation?
No. The 150 mg tablet and 50 mg capsule formulations of olaparib are not interchangeable on a milligram-to-milligram basis due to differences in how the body absorbs each formulation. You should strictly follow the prescription provided by your doctor and not switch between formulations without medical guidance.
