If you have any degree of renal impairment or end-stage renal disease on dialysis, there is no need to adjust the dosage. This recommendation was made based on safety and pharmacokinetic data from mostly observational studies.
If you have mild, moderate, or severe hepatic impairment (Child-Pugh Class A, B, or C), there is no need to adjust the dosage for sofosbuvir-velpatasvir. However, if you have decompensated cirrhosis and are receiving sofosbuvir-velpatasvir and ribavirin, clinical and laboratory monitoring is recommended. (4)
