What is Panovir?

Panovir is a generic fixed-dose combination tablet of Sofosbuvir 400 mg and Velpatasvir 100 mg โ€” a pangenotypic direct-acting antiviral regimen โ€” manufactured by Incepta Pharmaceuticals Ltd. under WHO-GMP certified conditions in Bangladesh. How Sofosbuvir and Velpatasvir Work

Hepatitis C virus depends on two viral proteins to replicate: NS5B polymerase, which copies viral RNA, and NS5A, which coordinates RNA replication and assembly of new virus particles.

Sofosbuvir is a nucleotide analog that is converted inside cells to its active triphosphate form, which is incorporated into viral RNA by NS5B polymerase and acts as a chain terminator, halting replication. Velpatasvir separately inhibits NS5A, blocking a different stage of the viral lifecycle. Together, this dual mechanism gives pangenotypic coverage across HCV genotypes 1โ€“6. In the pivotal ASTRAL trials, this regimen achieved sustained virologic response (SVR12, considered a cure) in over 95% of patients without cirrhosis or with compensated cirrhosis.

What to Expect: First 30 Days

Unlike older interferon-based HCV therapy, this regimen is generally well tolerated โ€” most patients continue normal daily activities throughout treatment.

  • Before starting: HCV genotype and viral load, liver function, and Hepatitis B screening (HBsAg and anti-HBc) are required โ€” HBV can reactivate during DAA therapy in co-infected patients.
  • Weeks 1โ€“2: Mild fatigue, headache, or nausea may occur but rarely require dose changes.

How to Take Panovir

Standard dose: one tablet once daily, with or without food, for 12 weeks (without cirrhosis or with compensated cirrhosis) or 24 weeks with ribavirin (decompensated cirrhosis, per your hepatologist). Completing the full prescribed course is essential โ€” stopping early risks treatment failure and drug resistance.

Why is amiodarone specifically a concern?
