Avalet(20mg)-Avatrombopag, also known as Doptelet, is a medication that increases platelet production in people with certain blood disorders. It is a small molecule that mimics the action of thrombopoietin. Thrombocytopenia is a hormone that stimulates the growth and development of platelets. Platelets are tiny blood cells that play a vital role in blood clotting. When you get a cut, platelets rush to the site of the injury and form a clot to stop the bleeding. By binding to  activate the TPO receptor on megakaryocytes, which are the cells in the bone marrow that produce platelets. This activation leads to the proliferation and differentiation of megakaryocytes, which results in an increased production of platelets.

Avalet(20mg) is an orally bioavailable, small-molecule drug. The good absorption of Avalet(20mg) by the gastrointestinal tract and its lack of metabolism by the liver make it a good option for patients with chronic liver disease. 

Dosage and Absorption for Avalet(20mg)-Avatrombopag| Dosage of Doptelet

  • Chronic immune thrombocytopenia (ITP): The starting dose of Avalet for ITP is 20 mg once daily.Avalet 20mg can be titrated up to 40 mg once daily or down to 20 mg three times weekly to achieve and maintain a platelet count of ≥50 x 109/L.

    Alternatively:

  • Thrombocytopenia in adults with chronic liver disease who are scheduled to undergo a procedure: The starting dose of Avalet(20mg) for thrombocytopenia in chronic liver disease is 60 mg once daily for 5 days for patients with a platelet count of <40 x 109/L, or 40 mg once daily for 5 days for patients with a platelet count of 40 to <50 x 109/L.

Food does not affect the absorption of Avalet. However, Avalet(20mg) should not be taken with grapefruit juice, as this can increase the blood levels of Avatrombopag and increase the risk of side effects.

CYP3A4 and CYP3A5 metabolize Avalet(20mg). However, it is not a significant inducer or inhibitor of these enzymes. Therefore, Avatrombopag is unlikely to interact with other medications that are metabolized by CYP3A4 and CYP3A5. The primary excretion route of Avatrombopag is the feces. The elimination half-life of Avatrombopag is approximately 16-19 hours.
