Three of the newest targeted cancer drugs in the United States now have generic versions made in Bangladesh. They are repotrectinib (brand name Augtyro), fruquintinib (Fruzaqla) and quizartinib (Vanflyta). Everest Pharmaceuticals makes them under the names Repotrex, Fruquin and Quizar.
For many patients, the original brands cost too much, or local pharmacies do not stock them. In these cases, new generic oncology medicines from Bangladesh can make treatment possible. However, an oncologist must prescribe them, and they must arrive through a legal import route.
This guide covers what each medicine treats and what the trials showed. It also explains the dose, the main side effects and how named-patient supply works.

Why Bangladesh can make generics of newly approved cancer drugs
The World Trade Organization’s TRIPS agreement gives least developed countries (LDCs) a special exemption. Under it, they do not have to grant or enforce patents on medicines. In 2015, the WTO extended this exemption until 1 January 2033.
Bangladesh is still an LDC. As a result, its companies can legally make generic versions of medicines that are still under patent in the US and Europe.
Bangladesh was due to leave the LDC group on 24 November 2026. In 2026, two UN bodies recommended a three-year delay to November 2029. The UN General Assembly makes the final decision.
Keep one point in mind, though. The exemption applies inside Bangladesh only. The medicine is not patent-free in the patient’s own country, so every order must follow that country’s import rules.
When you assess a generic from Bangladesh, check three things:
- Manufacturer: Everest Pharmaceuticals Ltd. makes Repotrex, Fruquin and Quizar in a WHO-GMP certified facility.
- Active ingredient: each product has the same active ingredient at the same strength as the original brand.

Repotrex 40 mg: generic repotrectinib (Augtyro)
What repotrectinib treats
Repotrectinib belongs to a group of drugs called tyrosine kinase inhibitors (TKIs). The FDA approved it as Augtyro on 15 November 2023. It treats adults with advanced ROS1-positive non-small cell lung cancer (NSCLC).
Patients can use it as their first ROS1 drug. They can also use it after another ROS1 drug has stopped working. In addition, the FDA gave it an accelerated approval on 13 June 2024 for NTRK gene fusion-positive solid tumours in patients aged 12 and over. Read more in our article on NTRK fusion cancers.
Older drugs such as crizotinib often fail because the tumour develops resistance mutations. For example, the ROS1 G2032R mutation blocks crizotinib. Scientists designed repotrectinib to keep working against mutations like this.
Trial results (TRIDENT-1)
The TRIDENT-1 trial tested repotrectinib in ROS1-positive NSCLC. In patients who had never taken a ROS1 drug, 79% responded. Their responses lasted a median of 34.1 months.
Patients who had already taken one ROS1 drug also benefited. In this group, 38% responded, and responses lasted a median of 14.8 months.
Dose
The usual dose is 160 mg, which is four 40 mg capsules. Patients take it once a day for the first 14 days. After that, they take 160 mg twice a day. You can take it with or without food.
At the full twice-daily dose, a month of treatment uses 120 capsules of Repotrex 40 mg (generic repotrectinib).
Augtyro side effects
Dizziness was the most common side effect in the trial. Other common ones were:
- changes in taste
- tingling or numbness in the hands and feet (peripheral neuropathy)
- constipation and shortness of breath
- problems with balance, memory or concentration
- tiredness and muscle weakness
Dizziness and balance problems can make driving unsafe. For this reason, patients should see how the drug affects them before they drive.
Fruquin 5 mg: generic fruquintinib (Fruzaqla)
What fruquintinib treats
Tumours need new blood vessels to grow. Fruquintinib blocks the receptors that send this signal (VEGFR-1, -2 and -3).
The FDA approved it as Fruzaqla on 8 November 2023 for adults with metastatic colorectal cancer. Patients must already have had the standard treatments first. These include chemotherapy with fluoropyrimidine, oxaliplatin and irinotecan, and an anti-VEGF drug. They also include an anti-EGFR drug if the tumour is RAS wild-type and the doctor thinks it is suitable.
Fruquintinib is a capsule that patients take at home. So it gives them a treatment option that does not involve chemotherapy.
Trial results (FRESCO-2)
The FRESCO-2 trial included 691 patients. Their cancer had grown despite standard treatments, including trifluridine/tipiracil or regorafenib.
Patients on fruquintinib lived a median of 7.4 months, compared with 4.8 months on placebo. In other words, fruquintinib cut the risk of death by 34% (hazard ratio 0.66).
Dose
The usual dose is 5 mg once a day for 21 days, then 7 days off. Each 28-day cycle then starts again. One pack of Fruquin 5 mg holds 21 capsules, so it covers one full cycle.
Fruzaqla side effects
Common side effects include:
- high blood pressure
- hand-foot skin reaction (red, painful palms and soles)
- protein in the urine
- a hoarse voice
- stomach pain, diarrhoea and weakness
Your doctor should check your blood pressure before you start. After that, they should check it regularly, along with the protein level in your urine.
Quizar 17.7 mg: generic quizartinib (Vanflyta)
What quizartinib treats
Quizartinib is an FLT3 inhibitor. The FDA approved it as Vanflyta on 20 July 2023 for adults with newly diagnosed acute myeloid leukaemia (AML). The leukaemia must be FLT3-ITD positive on an approved test.
Doctors first give it with standard induction chemotherapy (cytarabine plus an anthracycline). Next, they give it with consolidation chemotherapy. Finally, patients take it on its own as maintenance. However, the FDA has not approved it as maintenance after a stem cell transplant.

Trial results (QuANTUM-First)
The QuANTUM-First trial added quizartinib or placebo to chemotherapy. Patients who took quizartinib lived longer (hazard ratio 0.78).
People often compare quizartinib with midostaurin, an older FLT3 drug. Midostaurin works with chemotherapy for FLT3-mutated AML in general. Quizartinib, by contrast, targets the FLT3-ITD type. In the US, patients can also take it on its own as maintenance. Your oncologist will choose between them based on the mutation and your heart health.
Dose
- Induction: 35.4 mg (two 17.7 mg tablets) once a day on days 8 to 21 of “7+3” chemotherapy.
- Consolidation: 35.4 mg once a day on days 6 to 19 of high-dose cytarabine, for up to four cycles.
- Maintenance: 26.5 mg once a day for 14 days, then 53 mg once a day, for up to 36 cycles.
Your doctor will lower the dose if the heart’s QT interval gets longer. They will also lower it if you take certain other drugs (strong CYP3A inhibitors). Doctors use Quizar 17.7 mg (generic quizartinib) for induction and consolidation.
Heart monitoring and side effects
Vanflyta has a boxed warning, the FDA’s strongest. It can lengthen the QT interval, cause a dangerous heart rhythm (torsades de pointes) and lead to cardiac arrest.
For this reason, you need an ECG before treatment and regular ECGs during it. Your doctor will also keep your potassium and magnesium levels normal. In the US, only a special safety programme (REMS) can supply the brand. Anyone taking Quizar needs the same checks.
Price: Augtyro, Fruzaqla and Vanflyta vs the Bangladesh generics
The original brands are expensive in the US. According to Drugs.com, 120 capsules of Augtyro 40 mg cost about US$15,600. That covers one month at the full dose. Fruzaqla 5 mg costs about US$26,900 for 21 capsules, which is one cycle. Similarly, Vanflyta costs about US$611 per tablet, or about US$17,100 for 28 tablets.

How to import these medicines legally
Import rules depend on the country where the patient lives. Many countries have a named-patient or special-access route. Under this route, a doctor can obtain an unregistered medicine for one specific patient.
For example, the UK has an unlicensed medicines route, and Australia has its Special Access Scheme. On the other hand, some countries restrict or ban personal imports. In the United States, the FDA generally does not allow imports of unapproved versions of medicines. Therefore, ask your oncologist to confirm the right route before you order.

- Your oncologist confirms the diagnosis and the biomarker, then writes a prescription. Repotrex needs ROS1 or NTRK fusion results, Fruquin needs a record of past treatments, and Quizar needs an FLT3-ITD result.

Frequently asked questions
Is it legal to import generic Augtyro, Fruzaqla or Vanflyta from Bangladesh?

Why does Quizar need an ECG?
Quizartinib can lengthen the QT interval, which affects the heart’s rhythm. An ECG shows whether it is safe to start. It also shows whether your doctor needs to change the dose later.
Can Repotrex be used after crizotinib or entrectinib stops working?
Yes. Repotrectinib works in ROS1-positive NSCLC after another ROS1 drug has failed. In TRIDENT-1, 38% of these patients responded.
